The explosion that killed 4 crewmen on the Missileer 21A carrier R108 Yeoman was caused by a failed coolent fan that allowed heat to build up in a key power junction. DDS Engineering say that the accident could happen again because of a flaw in the power junction design and are now urgently looking into redesigning the power junction.
Until then engineering crews have been instructed to install extra heat monitors and to prioritise maitenance and monitoring of cooling systems.
