Explosion on Missileer kills 4

There has been an explosion on the Missileer 21B carrier R108 Yeoman which killed 3 crew outright and a 4th died afterwards. The ship, which was in orbit around Terra, was taken out of service immediately after the explosion in it's weapons bay. At first it was feared a TPM-2 had exploded because the epicentre was near one of the launchers but later analysis showed that the explosion was caused by an overheating power junction. Indeed the TPM-2s did not explode despite the force and heat of the initial explosion, a testament to work done on the missile to prevent these kinds of chain reactions as otherwise the ship could have been destroyed.

All Missileers are now being checked to make sure power junctions are OK. A DDS Engineering team is currently on the Yeoman seeing if the overheating was caused by a design flaw or a failure.
